New Mobile Technology Access and Payment Option Request forms required

Good Morning,

Pursuant to the Mobile Technology Allowance Payment Options   a new Mobile Technology Access and Payment Option Request form (MTAPOR) will be required by July 1st  (FY 2016) for employees that are receiving monthly mobile technology allowances.   If these payments are being processed within your department, you will need to collect the new MTAPOR form and keep it with your payment documentation.  If the payment allowance is being processed centrally by Accounts Payable in the Business Affairs Office, you will need to send the new completed and signed MTAPOR form to Accounts Payable (A/P) apbao at<mailto:apbao at> by June 15, 2015.  If A/P does not receive a new MTAPOR form, allowance payments will discontinue beginning with the July 2015 payment.  Please include your contact information when you send in your form(s) so we can contact you if we need additional information. This year there is an electronic version of the MTAPOR form.  The electronic form is designed to allow completion online and route the form for approvals utilizing email.  The form will retain history of all approvals and if any change takes place the form will be rerouted for approval by all parties.  Here is  a link to the electronic form: .

If you use the electronic version, please remember that if you want A/P to process a recurring payment, you will need to forward the email indicating all approvals have been completed to apbao at<mailto:apbao at> .  Otherwise, A/P will not be notified and will assume the form is being processed by the department.
